Proudly Serving Ocean, Monmouth & Mercer County

We’re continuously growing our knowledge on providers and policy in order to best serve our individuals through Support Coordination. We believe in taking the time to educate our families to make them feel comfortable and confident in a system which might be new to them. We believe a Support Coordinator's job is not only to find services for individuals, but to take the time to guide them to additional resources that they may qualify for.

This is a program that would not draw from DDD dollars. Click "PPP & PCA" to learn how to apply.

Food Assistance

Sometimes people feel they shouldn't need help putting food on the table, but sometimes making ends meet is a struggle.  If you are not sure how or if you can pay for your next meal or next week's groceries, NJ SNAP can help.  NJ SNAP, formerly Food Stamps, is New Jersey's Supplemental Nutrition Assistance Program that can help low-income families buy the groceries they need to eat healthy. Click "NJ SNAP" to learn how to apply.

Special Needs

The NJ FamilyCare Program (Medicaid) provides dental benefits so members can have regular dental check-ups, cleanings and needed dental treatment. Cavities and gum disease are preventable dental problems. Dental exams twice a year, brushing your teeth daily with toothpaste containing fluoride and flossing are good habits for a healthy smile. If you are enrolled in the Fee for Service (non-HMO) program, your Medical Assistance Customer Center (see list) can assist you in locating a dentist. If you are enrolled in an HMO, you can call the dental number on your ID card or use the list of dentists on your health plan’s website to locate a dentist.
